Education Goals
Dan decided to go back to school to be a part of the social work field. Ultimately, his goal is to have a job helping his peers who also have struggled with substance use and mental health disorders. He is already a Certified Peer Support Specialist in the state of Utah, and eventually wishes to receive his Advanced Substance Use Disorder Counselor (ASUDC) certificate as well. Graduate and Mentor of MRT, Certified Facilitator or SMART Recovery, Member of CAB Committee for 4th St Clinic, Member of Salt Lake Community College Social Work Association, Volunteer for USARA,
has taken Safe Zone Training, and does outreach with Legacy Initiative.

Financial SMART Goals
Long-Term Goal Statement:
My long term goal is to have saved $4,000.00 to buy a running motorcycle I am not to concern of the make or model just as long as it is 88 cubic inch engine or higher.
Mid-Term Goal Statement:
My mid-term goal is to have worked and saved $2800.00 to buy a running vehicle by January 1st 2018.
Short Term Goal Statement:
My short time goal is to pay the fine I owe the state of Washington by April 1st of 2017 and have my driver’s license reinstated and in my possession.

My long term-goal is to Finish Mastery Level by March 30th of 2019 and be able to facilitate one of the beginning levels at the retreat in April of 2020

My mid-term Goal is to go to the next level of this Journey and that will be Alchemy which is also a year commitment. I will start this in March of 2018 and complete that level by March 30th of 2018

I will attend the 2nd part of a journey I have started and go to Path Ways in March of 2016 that is the 1st retreat of a yearly commitment.
